Subject: [PATCH] Add GitHub Actions CI workflow

Run typecheck, lint, test and build on every pull request and on
pushes to main. Changes that touch only Markdown, docs, editor or
Claude Code settings, LICENSE or .gitignore skip the workflow.

Node version comes from .nvmrc; npm dependencies are cached. A newer
run on the same ref cancels the one in progress.
