docs(pm-dispatch): point the governed verdict at SKILL.md and bound the hand-off reader

The check-governed-merges verdict ends with the four pieces only on the
over-3000-line branch, so the landing clause now points at SKILL.md for
them. The H43/H48 reader line now says it reads governed Tier H PRs only
and that an over-line PR with no governed path is read by the seat itself.
