feat: hunk-level staging — diff parsing (foundational)

Scope
Foundational for patch-level staging. Extend the existing diff/word-diff parsing to expose hunk boundaries per file. No UI changes in this issue.

Acceptance criteria

• Unit tests confirm hunk boundaries are parsed correctly for representative diffs (adds, deletes, mixed context).
