fix(engine): advance DM unread cursors on participant rows - #596
A DM participant's inbox unread count follows dm_participants.last_read_id. Marking a message read, or acknowledging its delivery, moves that cursor forward. Later inbox reads stay cleared, and an older message id does not move the cursor backward.
Channel unread still uses channel_members.last_read_id. A DM has no channel membership row, so the inbox reads the participant cursor for DMs.
Migration 0078_dm_participant_read_cursor.sql adds the nullable cursor. Existing conversations stay unread until the next mark-read or delivery acknowledgement.

Walkthrough
The change adds a DM read cursor to dm_participants. Read operations and delivery acknowledgements advance the cursor, and inbox unread counts use it to determine which DM messages remain unread. Channel unread counts continue to use the channel membership cursor. Both inbox queries compare message IDs by length and then text.

A migration and schema field add a nullable participant read cursor and backfill it from DM read receipts. markRead advances the cursor with the existing atomic read-state writes. Migration and atomicity tests cover the cursor and its updates.
DM unread counts use the active participant cursor. Channel unread counts continue to use the channel membership cursor. Both use length-then-text ordering for message IDs. Conformance tests cover DM reads, cursor ordering, and channel unread behavior.
Advance cursors during delivery acknowledgement packages/engine/src/engine/delivery.ts, packages/engine/src/__tests__/atomicity.test.ts, packages/engine/src/__tests__/conformance/dmUnreadCursor.test.ts
Delivery acknowledgements batch delivery and read-state writes. Cumulative acknowledgements process batches of up to 50, advancing the sequence cursor on the final batch. Tests cover DM unread counts after acknowledgment, dead-lettered deliveries, and cursor-update failures.
AgentWorkforce/relaycast#580: This PR adds and advances dm_participants.last_read_id for DM unread counts, addressing the issue’s cursor mismatch.

Full details: Linked Issues check
Explanation
Issue #580 requires DM unread counts to decrease after a read action. markRead updates the active participant's dm_participants.last_read_id in the atomic write batch. getInbox counts DM messages after that cursor. The cursor moves only forward. Delivery acknowledgements use the same cursor path. Migration 0078_dm_participant_read_cursor.sql adds the nullable column and seeds it from existing DM read receipts. Conformance and atomicity tests cover DM clearing, acknowledgements, ordering, non-regression, and rollback behavior.
Full details: Out of Scope Changes check
Explanation
The changed files support the DM unread-cursor objective in #580. The schema, migration, receipt path, delivery path, inbox query, and migration and conformance tests implement or verify that behavior. The shared length-then-lexical cursor comparison also preserves correct channel unread behavior and has channel regression coverage. Changelog updates document the related fix. No unrelated public API or feature area is changed.

Summary
DM unread counts now use each participant’s last_read_id. Reads and delivery acknowledgements move that cursor forward without moving it backward.
DM unread counts follow each participant’s read cursor.
Inbox counts order message IDs by digit length before text.

Reviewed with an eye for abuse, since #580 came from an outside reporter. I found no exploit path. The issue reads like a genuine bug report, and the PR avoided its riskiest suggestion: option 3, auto-inserting channel_members rows for DM participants, would have mixed DM participants into channel-membership authorization checks such as markRead's first lookup.
What I checked:
Cursor scope.advanceDmParticipantReadCursor only updates the caller's own dm_participants row (agent_id = caller AND left_at IS NULL). channelId always comes from a server-side lookup: the message row in markRead, and the deliveries⋈messages join in the ack paths. A caller can't move another participant's cursor or point the update at a conversation they aren't in.
Authorization.markRead already rejects non-members and non-participants before any write, so the new statement adds no new reach. Ack paths go through getOwnedDelivery / node + agent ownership as before.
Dead-letter gating. Gating the receipt and both cursors on the pre-ack status fixes a small existing gap: before, acking a dead-lettered delivery still wrote a receipt and moved the channel cursor. Good catch, and it's tested.
No leakage. No route serializes raw dm_participants rows, so last_read_id doesn't show up anywhere new.
Two inline notes. The batch-size one is worth a look before merge; the other is non-blocking.
Migration 0078 seeds each DM participant cursor from its newest read
receipt, so already-read conversations do not come back unread.
Channel unread shares the length-then-text cursor predicate.

Add a DM-backed cumulative-ack test that checks the cursor after each chunk.
The chunked backlog test at packages/engine/src/__tests__/atomicity.test.ts:231-260 builds its backlog from channel messages. The DM cursor update matches no row for a channel message, so this test never reads dm_participants.lastReadId. A change that keeps the DM update statement but suppresses its effect in intermediate chunks, for example by gating it on the last chunk, would pass this test and the single-delivery DM tests. A change that removes the DM update call entirely would still fail the existing test, because the injected failure would never fire.
Add a DM backlog that spans more than DELIVERY_EXPIRY_BATCH_SIZE deliveries. Assert the cursor after a failed second chunk and again after retry. A final-cursor assertion alone does not detect a skipped intermediate write.

Add a DM-backed cumulative-ack test using sendDm to create a backlog larger than
DELIVERY_EXPIRY_BATCH_SIZE, then acknowledge it with ackDeliveriesUpToSeq while
injecting a failure in the second chunk. Assert dmParticipants.lastReadId
remains at the first chunk’s final message after failure, then retry and assert
it advances to the backlog’s final message.
