Reconciles the single authoritative current-execution document after #406:

• records exact Tighten maintenance structure and deployment hygiene #406 production deployment evidence;
• records the new documentation map / historical-ledger boundary;
• records simplified fail-closed branch pruning;
• records the new Vercel master/preview ignored-build policy;
• keeps Fix secondary workspace horizontal overflow #404 as the latest product-behavior code checkpoint while distinguishing Tighten maintenance structure and deployment hygiene #406 as repository/deployment-control maintenance.

Scope boundary

Documentation only. No runtime/product/UI/domain behavior change. No Playwright.
