Closes #322
Depends on #334 (Terraform: enables optional TOTP MFA on the Cognito user pool). That PR is split out separately so the small infra config change can be reviewed/merged independently — this PR is app logic only and needs #334 merged/applied before the live flow is testable end-to-end.

Unit tests pass (backend + frontend).
Live end-to-end enrollment (scan QR → verify code → log back in) can't be exercised until #334 is applied to the real Cognito pool.
