Stale mentions block every later content save of the page.
assertMentionTargets checks every mention in the submitted content, not only new ones. A mention can become stale after the page is saved:
- A mentioned member is removed from the project.
- A mentioned task is deleted or moved to another project.
The content JSON keeps that mention node. After that, every content save returns 400 with "A mentioned member is not on this project" or "A mentioned task is not in this project". Autosave stops working for the page until a user finds and deletes the stale mention by hand.
resolveMentions (Lines 249-273) marks such mentions as unavailable. That shows stale mentions are an expected state.
The same check also rejects team owners, admins and team leads who are not in project_members, although verifyProjectAccess lets them use the project.
Apply the target check only to diff.insertedMemberIds and diff.insertedTaskIds. Keep the mentions that were already stored.

Document search always fails because the ESCAPE clause is invalid.
In the JS template literal, '\\\\' becomes '\\' in the SQL text. PostgreSQL uses standard_conforming_strings = on by default. With that setting, '\\' is a two-character string. PostgreSQL then rejects the query with "invalid escape string" because a LIKE escape string must be empty or one character long. So every request to GET /project-documents/search that has at least 2 characters returns a 500 error.
Backslash is already the default LIKE escape character, and escapeLike uses backslash. So you can drop the clause. You can also emit a single backslash instead.

Fix the CI failure: locale values are copied from English.
The strict locales:untranslated check fails because some values equal the English strings. This breaks npm run build and the Docker build.

activeRef gets the new updated_at only when React renders, so handleRename can send an old timestamp and cause a false conflict.
handleSaved assigns activeRef.current inside the setActive updater. The autosave hook calls setSaveState('saved') on the same fiber just before onSaved. Because that update is already pending, React does not run the setActive updater right away. It runs the updater during the next render.
handleRename reads activeRef.current.updated_at as soon as await flush() resumes. That code runs in a microtask, before React renders. Here is the trigger:
- Type in the active page.
- Within 800 ms, rename the same page from the sidebar.
flushsaves and moves the server row to a newupdated_at.- The rename request sends the old
expected_updated_at. - The server returns 409.
setConflictDocumentthen blocks editing and shows the "This page was updated" banner, even though no other user saved anything.
The updater also mutates a ref inside a state updater, which static analysis flags. Update the ref synchronously, then pass the new state to setActive.

Serialize flush calls so that two saves of the same page cannot conflict with each other.
flush does not check whether a save is already running. updatedAtRef changes only after the first request returns. Here is the trigger:
- Save A starts with
expected_updated_at = U0. - While A is still in flight, the user keeps typing. The 800 ms timer fires, or
openDocument,handleCreateorhandleRenamecallsflush(). - Save B starts with the same
U0and different content. - A succeeds, and the server row moves to
U1. - B gets 409.
blockedRefbecomestrue, and the page shows the conflict banner. Further edits stop saving until the user reloads.
This happens whenever the save round trip takes longer than the debounce. That is normal on slow networks during continuous typing.
To fix this, keep the in-flight promise. A later flush must wait for it and then save the latest payload with the new updatedAtRef.
