Adds a seventeenth project-authored local fixture in which the host destination mapper writes an in-memory marker and throws before an action plan or receipt exists. The builder and quickstart distinguish zero tool callbacks from changed mapper state and direct hosts to reconcile their own state. No human approval, provider call, delivery evidence, or production safety claim is implied. An unknown receipt may follow a side effect and must not be silently retried.
Local verification: 13 focused tests passed; generated rehearsal report and content-based asset URL checks passed;
