fix(provider): reconcile orphaned runtime turns

Summary

• reconcile persisted active-turn claims after projection truth says the turn is terminal
• require that no live provider session exists for the thread
• clear only the orphaned activeTurnId and mark its binding stopped
• preserve provider identity, runtime mode, adapter key, and resume cursor
• record a timestamped orphan-reconciliation activity

Safety boundary

This is a direct, independent Cafe dev review unit. It does not call stopSession and cannot interrupt provider work. A live provider session always prevents reconciliation.

This PR changes only ProviderSessionReaper and its focused tests. It contains no WebSocket, Auto Nudge, atmosphere, renderer, or provider-version work.

Verification

• focused reaper tests: 11 passed
• full test graph with TZ=UTC: 9/9 package tasks
• server: 169 files, 1,311 passed, one skipped
• full typecheck: 9/9 packages
• exact-file lint and format passed
• git diff --check passed
• production and regression-test patch IDs match the current Club implementation
• controlled scope check found zero paths outside the two reaper files
