Responses WebSocket continuations can lose connection-local history when retries, provider fallback, or transport migration send previous_response_id to a different physical upstream connection.
Bind protected continuations to the live upstream socket's provider, endpoint, generation, and complete header fingerprint. Recheck access and limits, reject missing or changed bindings with a structured previous_response_not_found error, and prevent retries, fallback, hedging, Replay, and fake streaming for these turns. Wait for terminal delivery before closing, discard queued turns after a reset, and carry continuation failures into attempt-scoped finalization with once-only resource cleanup.
Validation on a standalone branch based directly on upstream dev (v0.9.8): production build, Biome check and autofix, TypeScript checking, and the full Vitest suite passed: 928 test files and 9,531 tests passed, with 13 tests skipped, using a 30-second per-test timeout. Autofix made no source changes. Regression coverage includes lost or mismatched bindings, routing and credential changes, retry suppression, terminal acknowledgment, and queued-turn cleanup.
Confidence Score: 3/5
Fix the lost protection for earlier local responses and the skipped endpoint circuit check before merging.
### Issue 1
server.js:478
`lastResponseWasLocal` forgets earlier local response IDs after a completed `store:true` turn. Create response A with `store:false`, complete response B with `store:true`, then reference A with `store:true`. The third turn gets no prior-local marker, and the retained socket now also has `connectionLocal:false`. The proxy can choose another provider or fall back to HTTP with A's connection-local ID, losing its history.
Track local response IDs and their socket generation instead of using only the last turn's storage mode.
### Issue 2
src/app/v1/_lib/proxy/forwarder.ts:1861-1867
The bound-endpoint path skips the endpoint circuit check in `getPreferredProviderEndpoints`. With `ENABLE_ENDPOINT_CIRCUIT_BREAKER` enabled, other requests can open the bound endpoint's circuit between WebSocket turns while the provider and vendor circuits remain closed. This branch still sends the continuation to an endpoint that the health guard has stopped other requests from using.
Check the endpoint's current circuit state and reject the continuation when it is open; do not switch endpoints.

Summary
The PR binds connection-local Responses WebSocket continuations to a live upstream socket.
Local Responses continuations stay on the upstream socket that holds their history.
The WebSocket bridge waits for each turn before advancing queued turns.
Unavailable continuations return a clear error and release their attempt resources.

Symptoms:
Responses WebSocket continuations can fail with previous_response_not_found
while the client connection remains open. Remote Compaction v2 can also
be recorded as successful after a structured WebSocket error.
Cause:
Logical session affinity does not guarantee reuse of the same physical
upstream WebSocket. Connection, routing, or credential changes can lose
connection-local history while requests still carry previous_response_id.
Structured WebSocket failures can also bypass final error accounting.
Solution:
Bind protected continuations to the live upstream connection, provider,
endpoint, generation, and complete request-header fingerprint.
Recheck access and limits, preserve prior storage semantics, and reject
invalid bindings with a canonical 400 error requiring full-context replay.
Prevent alternate routing, retries, HTTP fallback, hedging, Replay, and
fake streaming for protected continuations. Confirm terminal delivery
before closing, discard queued continuations, and preserve attempt-local
error accounting with idempotent resource cleanup.
The continuation path remains usable, but client disconnects during startup are recorded as 400 errors instead of client aborts. Correct the classification before merging or accept the bounded reporting impact.
