fix(adapters): admit core 0.12 - #570

Widen the Hex gen_agent requirement in claude, codex, anthropic, openai and ensemble to include ~> 0.12.0. Ensemble keeps 0.11 as its floor for GenAgent.Client.reset_session/2. minimum-core-check.sh now matches the requirement string alone, since mix format splits the claude and codex dependency tuple across two lines. README compatibility ranges updated.

The release PR for core 0.12.0 (#567) fails the Hex archives job:
