cmdutil.Confirm ignores a failed prompt write: an unseen (y/N) can count as consent #1077

Description

Problem

cmdutil.Confirm and cmdutil.ConfirmDeletion (internal/cmdutil/confirm.go) write the (y/N) prompt with fmt.Fprintf(io.ErrOut, …) and ignore the returned error. If stderr cannot be written (closed, full, broken pipe) while stdin is a terminal and delivers a y, the helper returns nil and the caller goes on to the write or delete — a question nobody saw counted as consent.

Found by Jane's LOCAL review of hadron-cli#1076 (probe: a TTY where only the (y/N) write fails and y arrives on stdin returned nil for both approve and decline). #1076 works around it locally for hadron automation approve|decline (a wrapper with a sticky stderr writer around Confirm); the shared helpers are untouched, so every other caller still has the gap.

Ask

Return the write error from both helpers (refuse, exit 1, "could not show the confirmation prompt … nothing was sent") and add one pin per caller class below. Once fixed, #1076's local confirm wrapper in internal/cmd/automation/mutate.go can be deleted.

Caller classes (63 call sites, grep -rn "cmdutil.Confirm(\|cmdutil.ConfirmDeletion(" internal): destructive deletes (memory rm, node rm, edge rm, object delete, asset rm, channel rm, schedule rm, webhook rm, secret rm, scope rm, org rm, aiconfig rm, mcpserver delete, app uninstall); consent and authority changes (app member add|remove, app update, app agent, org member, memory member|share|transfer|subscription|linkuser|encrypt, grant revoke, connection revoke, auth impersonate|token, user setroles); spec and node writes (node revision|merge|mint|import, spec supersede|replace|corpus, replace, coding fix, comment); team and host operations (team worker|role|attention, ticker, channel register, webhook rotate, run cancel, memory config-*, memory approve, asset public-link).

Acceptance (red first)

  • A TTY test double whose stderr fails only on the prompt, with y on stdin: Confirm and ConfirmDeletion return an error and the caller sends no request (one table-driven test over a representative command per class).
  • --yes is unchanged (no prompt, no write to check).

Out of scope

The wording of any prompt.

Attach for the gate (verified at hadron-cli origin/main).

    • Surface: internal/cmdutil/confirm.go: ConfirmDeletion (~l.26) and Confirm (~l.54) call fmt.Fprintf(io.ErrOut, …) and discard the error, so a failed prompt write plus a y on stdin returns nil. The Add hadron automation list|approval|approve|decline (#1071) #1076 workaround is stickyWriter and confirm() in internal/cmd/automation/mutate.go (~l.93-122).
    • Ask: both helpers check the prompt write; a failed write returns exitcode.Error (1) with "could not show the confirmation prompt (…); nothing was sent" and the action does not run. --yes is unchanged. The confirm()/stickyWriter wrapper in mutate.go becomes dead and is removed in this slice; Aborted. writes may stay unchecked (state that in the PR).
    • Callers: 63 cmdutil.Confirm* call sites; the helper change covers them. Also check the second helper confirmOverwrite (internal/cmd/node/import.go) and internal/cmd/agent/agent.go; if either has its own prompt write, fix it the same way or say why not.
    • Acceptance (red first): a new shared test double in internal/cmdutil whose stderr fails only on the prompt, y on stdin: both helpers return the error and no request is made (one representative command per class plus the helper unit tests); --yes still proceeds; removing the write check fails the test.

SPECS FIT, cli#1077 (task 01a1761cfa6475baaee6af0934a633dc; Patrick's attach and Xylon's gate request). Read the issue, the attach, and internal/cmdutil/confirm.go and confirmOverwrite (internal/cmd/node/import.go:858) on hadron-cli main. Searched the specs corpus (hrn:mem:hadronmemory.com:specs) by keyword, hybrid and regex for confirmation prompts, --yes, and consent before destructive actions. No code run.

    1. Governing contract: none in the corpus. Spec-clean. No spec node defines how the CLI asks for confirmation, so there is nothing to conflict with or to edit. The contract is the CLI's own (#129 destructive-op regime, as the code comment says): --yes skips the prompt, a terminal is asked y/N, a non-interactive caller without --yes is refused. The attach restores that intent (a refused-to-show prompt must not count as consent). I mint nothing and edit nothing.

    2. Scope: CLI-only. Server law is unchanged. Whether a deletion or an overwrite is allowed is decided by the server gates, not by the prompt, and the prompt is not a server authorization. agent rm and node import keep their existing server rules (e.g. cor:dmo:050 for agents). No MCP, GraphQL, portal or docs change follows. The server and portal have their own confirmation surfaces; this issue does not touch them.

    3. Ruling gap: none. The behavior asked for (fail closed when the prompt cannot be shown, --yes unchanged) follows from the existing rule that deletions are always explicit. No Holger question. Two details are implementation choices, not rulings: leaving the Aborted. write unchecked (harmless, the action has already been refused) and the exact error wording.

    4. Fit notes for the build.

    • Check the prompt write BEFORE reading stdin, in both helpers. Verified on main: both discard the fmt.Fprintf error, then read stdin.
    • confirmOverwrite delegates to Confirm (confirmed), so it needs no change of its own. The attach's agent.go check: I did not find a separate prompt write there; the builder should say so in the PR.
    • Remove the stickyWriter/confirm() wrapper in internal/cmd/automation/mutate.go only after the shared protection and its red test are in, as the attach orders.
    • A refusal must exit non-zero and send no request ("nothing was sent" must be true for every one of the 63 call sites; pin it with one command per class).

    5. Security routing. This is a client-side consent surface, not a server authorization gate: no tenant data, no permission check, no new input. Scoped §10 as Xylon proposed is enough; a security engineer reads the helper and the test for fail-closed behavior. No special security task beyond that. Not a spec task, so no Built line is owed from me at merge.

CHECKED — cli#1077. @shadowbrush Patrick's attach passes against current main b59fdd412d54a6faab1dbd46c4ca543f21887531; delegated Vera FIT returns spec-clean, CLI-only, no ruling gap. Bug type and cleared set.

Verified: internal/cmdutil/confirm.go discards the prompt write in both helpers before scanning stdin; node/import.go:confirmOverwrite delegates to Confirm, agent/agent.go deletion delegates to ConfirmDeletion, and automation/mutate.go contains the temporary stickyWriter/confirm wrapper. Those delegating callers need no separate prompt implementation. No server, portal, MCP, library or spec change is required.

Build boundary: check the prompt write before consuming stdin; return exitcode.Error (1) on failure. Preserve --yes, noninteractive refusal, negative-answer/EOF cancellation and existing prompts. Remove automation's wrapper only once shared protection exists, retaining its regression coverage. Leaving the Aborted. output unchecked is acceptable because cancellation has already refused the action; disclose that limit. Error text must not suggest a mutation was sent: command tests must prove no mutating request/action follows the failed prompt (ordinary prerequisite reads are not mutations).

Acceptance: red-first helper tests for BOTH helpers with prompt-only stderr failure and stdin y; prove no stdin read after failed write. Representative command per the four caller classes must send zero mutations on that failure; --yes proceeds without prompting and normal successful confirmation still works. Removing either write check must fail the corresponding proof. Independent QA runs the compiled CLI with a disposable/fake transport and no production actions.

Review schedule: T2 — author suite + green CI; separate cross-family BLIND and LOCAL engineer seats (both differ from author family, neither a change manager); QA: independent compiled CLI across the four caller classes, both helpers, refusal and positive controls; UX: waived, restores intended confirmation; §10: scoped fail-closed consent review at every head, including automation wrapper removal. No production, credential, tenant-isolation or server-authorization behavior is changed.

Closed by hadron-cli#1091, merged as 8a0e5851 (merge commit; reviewed head 39e79a4f is the second parent). Evidence at that head: §10 APPROVE, LOCAL APPROVE (Joost, cross-family), BLIND P2 withdrawn by the blind reviewer after Joshua's helper evidence, QA PASS plus addendum (real binary, pty y, /dev/full stderr), CI 4/4, 0 behind main.
