Re-register Hindsight tools for each replacement runtime and rebuild visibility from current session state. Restrict visibility changes to Epimetheus's canonical tool names, preserving every other active tool regardless of prefix or registration order. Add startup and tool diagnostics. Persist debug output to disk and cover replacement, visibility, and logging behavior.
