📦 Polish npm page and release v0.2.95

Summary

• redesign the Scout npm README with branded artwork, badges, product positioning, quickstart, routing guidance, package boundaries, and current-posture links
• set the npm package homepage to https://openscout.app and improve npm discovery metadata
• bump every canonical public release version source to 0.2.95
• add presentation tests for the homepage and branded README contract

The full local unit command reaches 567 desktop passes, then reports two @OpenTui native-module initialization errors in monitor.test.ts. The exact monitor failure reproduces unchanged on the v0.2.94 source/tag, so it is not introduced by this 14-file metadata/presentation patch. This PR will also receive a manually dispatched clean GitHub CI run before merge.

The Homebrew cask will be published after the signed and notarized 0.2.95 DMG exists so the cask can pin the final SHA-256.
