修复 issue #227
chapters 分层缓存补种逻辑的 segments 分支不再取运行态
cache_data["segments"]（内存形态随运行条件漂移），改用与 notes reader 同源的transcriber.segments.load_segments(file_path)——chapters 存储指纹自此永远可从磁盘复现，notes 生成不再确定性失败。生产实证（BV1fcHD6cEw3）：writer 存指纹
b5bebec0a91b，readerload_segments556 段复算ed62cc163258，MISMATCH → notes 任务 task_744c0ad4f5ea4a2c9539f7a00d510117 8ms 确定性失败。改动
transcription.py：删 2 行（cache_data["segments"] 分支）；dialogs 优先与 load_segments 兜底保留test_layered_cache.py：新增同构性回归测试（内存形态 ≠ 磁盘形态时，种子指纹必须 == notes reader 指纹）验证
部署后动作（合并部署后执行）
删 BV1fcHD6cEw3 的 llm_chapters.json → 重提重生成 chapters → 重试详细笔记，验证 #227 闭环。
